What is conservative dentistry and what does it consist of?

Conservative dentistry is a branch of restorative dentistry that consists of dental procedures and ancillary operations that allow the teeth to be treated by removing decayed dental tissue  such as the closure of the cavities formed or the recovery of enamel and dentin which are gradually lost with caries. Removal, shaping of the cavity and shaping of the filling are performed with the aid of burs mounted on rotating instruments

the importance of conservative therapy is not only linked to the treatment of caries , but also to subjects who suffer from bruxism and have worn or damaged teeth due to trauma .

Sometimes where the reconstructions require further support because the crown is highly damaged, it is necessary to use root pins that serve as support for the composites for the reconstruction to ensure a longer duration of the dental element, it is however advisable to create a capsule where the dental crown is severely damaged .

Dental sealings are always part of the conservative therapy and are sold in a fluid and transparent color form that are normally performed in the grooves of children's teeth and are used to prevent those teeth from being attacked by carious processes in the grooves that are areas where it is most difficult to clean.

Conservative therapy is an ideal solution to ensure an excellent therapeutic result

Conservative therapy is a perfect solution to remedy dental trauma , as well as caries , in all subjects: not only adults, but also children can be appropriately treated using new generation techniques . Especially with regard to the youngest patients, an ideal solution to guarantee an excellent therapeutic result is that of sealing .

These are carried out on the dental grooves in such a way as to prevent caries from attacking dental elements that have natural grooves in which there may be the accumulation of food of complex removal. The treatment for children is completely painless and always carried out in maximum safety,  the most complete oral hygiene is also guaranteed. The interventions at the level of the dental furrows are important, which are finely cleaned of the dirt that can accumulate and are stratified by means of special colorless fluid resins .

If the anchoring surfaces of the reconstruction were reduced, it is possible that the filling could detach. If, on the other hand, the residual walls of the tooth were very thin, there is a risk of fractures.

In the event that the caries is very extensive to affect the pulp , the need arises to have to devitalize the tooth; if instead it were only very close to the pulp, an isolation can be applied, however running the risk that the tooth goes into necrosis leading to a subsequent devitalization.

What are dental caries and what are the causes that lead to decayed teeth?

Caries is an extremely slow-moving dental infection triggered by the attack of some microorganisms that populate the oral cavity. It is a degenerative process that damages the hard tissues of the tooth, enamel and dentin and is caused by bacteria present in the oral cavity and which take refuge in the plaque and tartar inside the dental arch.

Caries  more common are the superficial ones and the deeper ones. In the case of superficial ones, modern conservative therapy techniques involve the removal of part of the dentin of the tooth in which there is caries and subsequently also of the enamel . We proceed with the filling using a suitable compound that will resemble the substances physiologically present in the teeth. These materials, which are sold in liquid or pasty form and therefore can be molded at will, are then hardened in a few seconds thanks to special photo-curing lamps.

The causes of tooth decay are multifactorial. The infectious process is caused by exogenous plaque deposits, improper nutrition, smoking and endogenous reduction of saliva, structure of the teeth.

Root canal therapy. Devitalizing a tooth means removing its pulp.

Devitalizing a tooth means removing its pulp. It is necessary to resort to devitalization when, for example, very important caries have already affected the pulp, when other injuries have caused an alteration of the same leading to death, when very extensive fractures of the tooth have also affected the pulp of the same.

The procedure of root canal therapy  that is, the devitalization consists, broadly speaking, in these phases:

  • Clean the canals of the tooth by carefully emptying them of nerves or infected tissue.

  • Causing a slight widening of the canals and disinfecting them thoroughly.

  • Seal them by filling them with specific materials . A devitalized tooth resists in the patient's mouth for many years. These teeth can again undergo treatments in the case of the formation of granulomas or infections. In particular cases it is useful to use the microscope and in these situations it is good to contact a dentist at the UFO Dental Clinic who can demonstrate specific preparation. Generally, where possible, it is preferred to preserve natural teeth for as long as possible and therefore we tend to use devitalization rather than extracting a tooth , especially when it comes to a tooth that is relevant in chewing.

The cases in which it is not possible to carry out devitalization are; When the root canals are difficult to access, when the tooth clearly demonstrates that it cannot be reconstructed, when there is a lack of suitable bone support.

What are dental inlays?

The alternative is the reconstruction performed in the dental laboratory: on the basis of an impression of the missing area the technician can build an inlay in composite resin or metal material which, once finished, is glued to the tooth, rehabilitating it.

Inlays and onlays are dental products used in the most advanced dental practices when the restoration of the dental crown is more complex than usual. They are part of conservative dentistry, they enter the scene when the alternative is to completely replace the crown of the tooth.

Inlays and onlays are an excellent alternative if the crown of the tooth is severely compromised by caries.

The filling is usually a good solution for the repair of small lesions of the dental crown

A dental crown severely damaged by caries or some other disease can be effectively repaired with the use of ceramic inlays or onlays.

The use of inlays and onlays is recommended to avoid complete replacement of the tooth crown and to protect the healthy part of the dental tissue from further deterioration. These ceramic supplements have excellent functional and aesthetic properties.

Inlay - we use the inlay when the lesion affects only one face of the upper or lateral tooth. By doing so, we will completely close the hole in the tooth and strengthen its structure .

Onlay - we resort to onlay when the lesion affects two faces of the upper and lateral tooth. By doing so, we will completely close the hole, reinforcing the structure and increasing the stability of the tooth.
